As a LPN/RN MDS Coordinator (Long-Term Care), you will play a vital role in ensuring the highest quality of care for our residents while promoting a positive and supportive care environment.
About the Role: As an LPN/RN MDS Coordinator, you will be responsible for coordinating the MDS process, ensuring accurate and timely completion of MDS assessments, and facilitating effective communication among the healthcare team. Your exceptional communication and organizational skills will enable you to work closely with residents, families, and staff to ensure that their needs are met and their concerns are addressed.
Responsibilities:
  • Coordinate the MDS process, ensuring accuracy and timeliness of assessments
  • Facilitate effective communication among the healthcare team, including physicians, nurses, and other caregivers
  • Review and analyze MDS assessments to identify trends, patterns, and areas for improvement
  • Develop and implement individualized care plans that meet the unique needs of each resident
  • Provide education and support to residents, families, and staff on MDS processes and long-term care requirements
  • Collaborate with the interdisciplinary team to develop and implement quality improvement initiatives
  • Maintain accurate and up-to-date records of MDS assessments, care plans, and progress notes
Requirements:
  • Current LPN/RN license in the state of Florida 1+ year of experience in long-term care or gerontology
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong organizational and time management skills
