Bilingual LTSS Care Coordinator (Field-Based)

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to support a defined caseload in the field, primarily residing in Hardee, Highlands, Hillsborough, Manatee, or Polk County, FL. This role travels to client sites, with some office attendance, under a hybrid/work policy. You will conduct assessments, develop Person Centered Plans, and coordinate care with medical directors and an interdisciplinary team to deliver integrated PH/BH/LTSS support.
