Opportunity Highlights Full-time outpatient practice Flexible schedule: Monday–Friday, 8-hour workdays, or Four 10-hour workdays per week Patient volume of approximately 16+ patients per day Limited call responsibilities: Primary Care call rotation by phone only with nurse triage support Collaborative, team-based care environment dedicated to the unique needs of older adults Competitive compensation and comprehensive benefits package with medical, dental, and vision coverage day one of employment State level student loan forgiveness opportunities Position Responsibilities The Nurse Practitioner will provide comprehensive outpatient care for an aging patient population, including: Conducting annual wellness visits and preventive health screenings Evaluating, diagnosing, and treating acute and chronic conditions Managing common geriatric concerns, including dementia and cognitive disorders, osteoarthritis and chronic pain conditions, urinary incontinence, and multiple chronic medical conditions Ordering and interpreting diagnostic tests and laboratory studies Developing and managing individualized treatment plans Collaborating closely with physicians, specialists, caregivers, and interdisciplinary team members Supporting patient goals while promoting independence, safety, and quality of life
