What You'll Do: Provide in-person and telehealth care Diagnose and manage acute and chronic conditions Develop personalized treatment plans (lifestyle, supplements, hormone therapy, medications)Educate patients on preventive and functional medicine Collaborate with a multidisciplinary care team Document in Next Gen EHRQualifications Active Oregon NP license (or ability to obtain)Board Certified: FNP, AGNP, or AGNP-PC2+ years of clinical experience preferred Interest in functional, preventive, or lifestyle medicine Strong communication and teamwork skills
