We are seeking a full-time Care Coordinator to support patients, families, nurses, pharmacies, and physicians in coordinating home infusion services. This role is responsible for managing referrals, scheduling visits, maintaining accurate patient information, and ensuring clear communication across the care team.
Responsibilities:
Coordinate patient referrals, nurse assignments, and visit schedules. Communicate professionally with patients, families, nurses, pharmacies, and physicians. Review referrals and ensure required documentation and information are complete. Match nurses to patients based on location, therapy, skills, and patient-specific needs. Verify required nurse training and coordinate training when needed. Maintain accurate scheduling, patient, and care coordination records. Document communications, concerns, and patient updates promptly. Coordinate required labs and follow up to ensure results are received appropriately. Communicate changes in patient care plans to nursing staff. Escalate patient care concerns to the appropriate clinical team members. Maintain patient confidentiality and follow HIPAA and company standards. Participate in team meetings and process improvement efforts.
Qualifications:
2+ years of healthcare or customer service experience. Associates Degree or higher is preferred Excellent communication and customer service skills. Strong organizational and time-management abilities. Comfortable using electronic health records and office technology. Strong attention to detail and ability to manage multiple priorities. Professional, compassionate, dependable, and team-oriented. Ability to work effectively in a fast-paced office environment.
