Home Health Registered Nurse

Hartford Healthcare is seeking a Central Shoreline HomeCare RN to provide high-quality, patient-centered nursing care in the home setting. In this role, you will perform comprehensive assessments, develop and implement care plans, administer medications and treatments, and coordinate with physicians, therapists, and families to support optimal outcomes. You’ll educate patients and caregivers, ensure accurate documentation, and promote safety and infection control. Provide comprehensive in-home nursing assessments and ongoing evaluations Develop, implement, and update individualized patient care plans Administer medications, treatments, wound care, and IV therapy per orders Educate patients and caregivers on disease management, medications, and safety Coordinate care with physicians, therapists, and interdisciplinary team members Document all care accurately in the EMR and maintain regulatory compliance Monitor patient progress, recognize changes in condition, and intervene appropriately Follow infection prevention, safety, and quality standards in the home setting Communicate effectively with patients, families, and care team Participate in ongoing professional development and quality improvement initiatives Home health nursing Patient assessment Care planning Wound care management IV therapy and medication administration Electronic medical records (EMR) Care coordination Patient and family education Infection prevention and control Time management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in CT BLS certification ACLS preferred Home health experience preferred Valid driver’s license and reliable transportation
