Work Environment: This position is based onsite within a home care setting, allowing our therapists to provide personalized and convenient care tailored to each patient's environment. As part of a supportive team of healthcare providers, you will have the resources and collaboration necessary to facilitate optimal recovery for your patients. The typical work schedule is Monday to Friday from 9 AM to 5 PM.

Key Responsibilities: Conduct comprehensive physical assessments to develop tailored treatment plans for patients. Implement a variety of therapeutic techniques, including manual therapy, exercise prescription, and modalities based on each patient's unique needs. Maintain a manageable patient caseload to ensure that each individual receives focused and effective care. Utilize a patient-centered methodology to foster motivation and facilitate compliance with treatment regimens. Document all patient interactions and treatment outcomes in compliance with best practices and regulatory standards using Electronic Medical Record (EMR) software, particularly Web PT.Continuously assess treatment progress, making necessary adjustments to optimize patient outcomes. Educate patients on best practices for health, wellness, and injury prevention. Collaborate effectively with other healthcare professionals, such as physicians and occupational therapists, to coordinate comprehensive care. Engage in ongoing professional development to stay abreast of the latest advancements in physical therapy techniques. Patient Population / Client FocusOur patient population includes individuals recovering from a variety of conditions, including orthopedic surgeries, sports injuries, and chronic pain issues. This role emphasizes the importance of creating customized rehabilitation programs designed to empower patients and promote their independence. Qualifications: A Bachelor's degree in Physical Therapy or equivalent education from an accredited program is required. An active Physical Therapist license with no history of sanctions or board proceedings. Strong knowledge of manual therapy techniques and modalities. Exceptional ability to design and implement effective exercise programs tailored to specific patient needs. Proficiency in using EMR software, specifically Web PT, for accurate documentation. A thorough understanding of current physical therapy trends and evidence-based practices.

Preferred Experience: Prior experience working in an outpatient clinic or home care environment is highly desirable. Demonstrated success in working collaboratively within interdisciplinary teams. Familiarity with advanced physical therapy techniques and technology.
