Key Responsibilities:

Provide in-home physical therapy services following the established Plan of CareAssist patients with therapeutic exercises, mobility training, and rehabilitation programs
Support senior care patients in improving strength, balance, and independence
Monitor, document, and report patient progress to the supervising Physical Therapist
Educate patients and caregivers on home exercise programs, safety, and fall prevention
Collaborate with physicians, RNs, and the interdisciplinary care team
Complete documentation using WellSky Kinnser EMR

Qualifications:

Active California Physical Therapist Assistant (PTA) license
Current CPR certification (AHA required)
Minimum 1 year of clinical experience (home health experience preferred)
Strong interest in home health, rehab, and senior care
Excellent communication and organizational skills
Valid driver's license, auto insurance, and reliable transportation
