Description:
We are looking for a Community Health Worker to support our clinic. The Community Health Worker/Certified Enrollment Counselor (CHW/CEC) is an important team member supporting the Outreach & Enrollment department, which is essential for achieving the dual mission of Full Circle Health. The CHW/CEC shall be accountable for:1) Professionally and compassionately serve to connect the community in the Family Medicine Health Center (FMHC) services area to local healthcare and community services and resources. 2) Responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. 3) Provide social support and informal counseling, advocate for individuals and community health needs, and help to provide services such as glucose test, and blood pressure screening. Community outreach, such as home visits and helping with health screenings at community fairs will be required. Qualifications
Education: High School Diploma (required)
Experience:
At least 2 years of experience in the medical field. Other: Our organization is a tobacco-free workplace. We require immunizations for all staff to include an annual influenza vaccine as a requisite for employment. Newly hired staff who are not able to be vaccinated due to medical reasons or sincerely held religious beliefs must apply for vaccination exemption prior to employment.
