We are seeking Primary Care Advanced Practice Providers for our Rhode Island locations. Schedule Monday-Friday, 8am-5pm (no nights or weekends)12-16 patients per day once fully ramped up (could start lower if new site) Patient population primarily Medicare (over age 65) but also includes Medicaid, with some sites having up to 25% under age 65 Team and Support Collaborative care team including 1 MD, APP, Scribe, MA, and shared RN/Social Work support Specialty support available through partnership with a telemedicine company, allowing rapid consultation with specialists such as Dermatologists, Cardiologists, and Gastroenterologists Utilization of EPIC EMR, managed by scribes and supported by an advanced algorithm to optimize patient care and reduce hospitalizations Call Schedule Rotated amongst the market, averaging 1 week every 3 months. As more clinics open, frequency will decrease. A call center is available to triage calls, ensuring they are limited and 100% remote.
