Job Summary: We are seeking an experienced Registered Nurse Clinical Manager to oversee clinical operations, support field clinicians, and ensure high-quality, coordinated patient care. This role works closely with physicians, patients, caregivers, and the interdisciplinary team to ensure appropriate care and strong clinical outcomes.

Responsibilities: Coordinate and oversee home health patient care Supervise and support clinical staff Develop, review, and update individualized plans of care Review referrals and assess patient care needs Coordinate physician orders, medications, and clinical services Lead case conferences and interdisciplinary care coordination Review clinical documentation for quality and compliance Support staff orientation, education, and clinical development Participate in QAPI and quality improvement activities Collaborate with the Director of Nursing and agency leadership

Qualifications: Current and active Registered Nurse license in California. Home health experience required 2+ years of clinical management or supervisory experience preferred Strong clinical judgment, leadership, and communication skills Current CPR certification Valid driver's license, auto insurance, and reliable transportation Join our team and help lead exceptional home health care while supporting clinicians and improving patient outcomes.
