TRU Community Care is hiring: Hospice Social Worker in Broomfield

JOB SUMMARY: The Social Worker is responsible for providing psychosocial services for the team caseload. Provides direct services to patient and family, collaborates with the other Interdisciplinary Team (IDT) members within the care team and other health care providers in various care settings.

ESSENTIAL DUTIES: Identifies the patient's and family's psychosocial, environmental, safety and developmental needs and participates in individualizing the patient's plan of care based on those needsIncorporates patient/family preferences and goals within the plan of careSupports and respects the patient's and family's unique spiritual and cultural beliefsFacilitates family meetingsIdentifies community resources for the patient and familiesParticipates in IDT, psycho-spiritual and other patient-related meetings; attends patient care conferences in LTC and AL facilities, as neededEDUCATION AND QUALIFICATIONSGraduate of an accredited Master's Degree in Social Work programOne year of experience in care setting that delivers end-of-life care, highly preferredMust possess CPR certification or be able to obtain within 60 days of hireMust successfully pass agency required background checks and drug screeningMust possess a valid driver's license, have reliable transportation, and auto insurance.
