The Resident Care Coordinator plays a vital role in managing and facilitating care for residents within healthcare settings. This position involves close collaboration with medium to large care teams to ensure seamless patient scheduling, effective communication across care providers and families, and diligent management of care plans and compliance standards.
Responsibilities:
Coordinate care activities and facilitate communication among care team members. Schedule patient appointments and manage calendar logistics. Serve as a liaison between patients, families, and healthcare providers. Develop and maintain individualized care plans. Allocate resources efficiently to meet resident care needs. Provide support and education to families regarding care procedures. Document all care-related interactions and maintain accurate records. Monitor compliance with healthcare regulations and organizational policies. Collaborate with a care team ranging from 6 to more than 16 members to ensure quality care delivery. Conduct assessments to evaluate resident needs and adjust care plans accordingly.
Preferred Qualifications:
Entry-level experience in healthcare coordination. Associate degree in Healthcare Administration or related field.\nCompany
