RN Case Management Manager

HCA Healthcare is seeking an RN Manager Case Management to lead a team of case managers supporting safe, efficient care transitions. In this role, you will oversee daily case management operations, including utilization review, discharge planning, and coordination of post-acute services. You will collaborate closely with physicians, nursing, social work, and ancillary teams to reduce readmissions, optimize length of stay, and improve patient outcomes. The ideal candidate brings strong clinical judgment, leadership experience, and familiarity with regulatory and payer requirements in an acute care setting. Lead and mentor the case management team to ensure effective care coordination and discharge planning Oversee utilization review processes to support appropriate level of care and resource use Collaborate with physicians, nurses, and ancillary staff to develop and implement patient-centered care plans Coordinate post-acute services, including home health, rehab, and community resources Monitor key metrics such as readmissions, length of stay, and throughput, and implement improvement initiatives Ensure compliance with regulatory, accreditation, and payer requirements Use EHR and data tools to track outcomes and document case management activities Participate in budget planning and manage departmental resources efficiently Develop and refine case management policies, workflows, and best practices Provide staff education and support professional development within the team Case management Utilization review Discharge planning Care coordination Electronic health records (EHR) systems Data analysis and reporting Quality and performance improvement Interdisciplinary team leadership Regulatory and compliance knowledge Budgeting and resource allocation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in state of practice BSN preferred ACM or CCM certification preferred BLS certification
