Job Description:
  • Case Manager - Nursing
  • Schedule8hr Days, Monday to Friday, 11AM to 7PMAssignment Details
  • Guaranteed Hours: 40 Weekly
  • Contract Weeks: 13 Requirements 2 years of Case Manager experience requiredRN License required, must be activeBLS certification required
  • Degree accreditation or 5 years of relevant experience required
  • Must provide demographic information upon submission
  • Floating between units and possibly to other facilities is required
Responsibilities:
  • Conduct assessments and develop care plans
  • Coordinate patient care activities and resources
  • Collaborate with interdisciplinary teams for optimal patient outcomes
  • Educate patients and families regarding care plans and resources
  • Monitor patient progress and adjust care plans as necessary
  • Document all findings and maintain accurate patient records
