⭐ About the Role: Arc Mercer is expanding its medical infrastructure with a new Medical Support LPN model designed to bring nursing closer to where people live, learn, and receive services. Instead of relying solely on a centralized Healthcare Center or traditional RN coverage, you’ll provide hands‑on clinical support across designated residential homes and day programs — especially those with higher medical needs. You’ll work under the oversight of our designated medical provider and serve as a proactive clinical resource for staff, individuals, and leadership. This is steady, meaningful nursing focused on prevention, follow‑through, and early identification.
🩺 What You’ll Do: Proactive Monitoring — Complete and track vitals, weights, blood sugars, and other ordered measurements. Identify changes from baseline and escalate appropriately. Medical Support — Provide enhanced clinical oversight to higher‑medical‑need homes. Track labs, referrals, imaging, follow‑ups, and outstanding recommendations. Appointment Preparation — Ensure individuals are clinically prepared for medical visits with accurate histories, vitals, medication lists, and relevant updates. Provider Follow‑Through — Review consultation notes, identify new orders, and help ensure labs, referrals, and medication changes are completed. Day Program Support — Provide nursing presence in designated day programs, respond to routine concerns, and coordinate with residential teams. Hospital/ER Follow-Up — Review discharge instructions, track new orders, and escalate unclear or concerning changes. Agency Triage — Respond to routine medical calls, provide guidance within scope, and determine when escalation is needed. Medication Support — Review new orders, identify discrepancies, reinforce medication procedures, and support improved eMAR/pharmacy workflows. Documentation & Tracking — Document vitals, observations, appointments, follow‑ups, and chronic‑condition monitoring within Arc’s systems.🕒 Schedule Options: Multiple🎓
What You Bring: Current New Jersey LPN license Experience with individuals with intellectual/developmental disabilities preferred Background in residential, community health, home care, primary care, or long‑term care Strong skills in chronic‑condition monitoring, vitals, medication terminology, and clinical documentation Ability to work independently across multiple locations Excellent communication, organization, and follow‑through Comfort functioning as a mobile clinical resource rather than a traditional nursing‑station role❤️ Why This Role Matters You’ll help shift Arc Mercer’s medical model from reactive to proactive — catching issues early, strengthening follow‑through, and improving health outcomes for adults with intellectual and developmental disabilities.
