Key Responsibilities: Conduct thorough initial assessments of residents to determine baseline mobility, pain levels, strength, and functionality, continually monitoring progress. Develop and implement customized treatment plans based on assessments, patient goals, and physician recommendations, ensuring alignment with best practice guidelines. Execute evidence-based therapy sessions, prioritizing interventions that enhance mobility, alleviate pain, and prevent future injuries. Educate residents and their families on therapeutic techniques and provide detailed instructions for at-home exercises to promote ongoing recovery. Collaborate with nursing staff, occupational therapists, and wellness professionals to create cohesive interdisciplinary care plans that holistically address residents' needs. Maintain accurate and timely documentation in strict accordance with Medicare and state regulations to ensure compliance and deliver quality care. As a Physical Therapist, your role will involve not only addressing the physical needs of these patients, but also understanding their emotional and psychological states to provide comprehensive support.

Qualifications: Active New York State licensure as a Physical Therapist is required. A Bachelor's degree in Physical Therapy from an accredited program is mandatory, with a Doctor of Physical Therapy (DPT) preferred. Strong interpersonal and communication skills along with clinical reasoning capabilities are essential for effective patient interaction and teamwork.
