Job DescriptionJob DescriptionJOB SUMMARY: The Case Management Supervisor (reporting to the Director of Social Services) has responsibility for the following:
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Directly supervise the Case Managers* Provide orientation and initial training to new Case Managers, Social Work interns and other social services staff as assigned.* Review daily case notes in CARES and ensure that Case Managers input case notes within 24 hours* Monitor incident reports and ensure that follow up case notes are being attached* Approve documentation in all charts* Identify and coordinate the training and educational needs of the Case Management staff* Facilitate groups that will address the needs of the clients* Provide support to clients who are struggling to follow up on their ILPs* Cover caseloads when necessary* Document client engagement in CARES* Audit the clients' charts to ensure compliance* Participate in case conferences, team meetings, staff training, community meetings, and staff meetings* Approve staff time cards* Conduct employee evaluation* Perform other duties as assigned.
JOB QUALIFICATIONS
Educational Requirements:
  • Master's Degree in Social Work (Licensed Master Social Worker) or other related fieldsExperience:* Minimum of 3 years of clinical experience* Experience working with the mentally ill and/or homeless population* Flexible and comfortable in nontraditional setting* Excellent oral and written communication skills* Proven supervisory skills (in a supervisory capacity for a minimum of 3 years)
