Nephrology Nurse Practitioner

Our client is launching an innovative pilot program to transform how Nurse Practitioners (NPs) deliver care to patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). We're looking for dynamic, forward-thinking NPs to help pioneer this new model of primary care providing proactive, integrated services alongside nephrology teams. (One NP focused on ESKD care and another NP focused on CKD care) What You ll DoAs a Nurse Practitioner you ll serve as a key provider for high-risk patients in either CKD or ESKD stages.
