RN Care Transitions Leader - Rehab to Home

Clermont Park is seeking an experienced RN to lead our rehab neighborhood as Care Transitions Manager. You will guide residents through transitions from skilled nursing to home, partnering with families and providers to support a confident, well-coordinated plan. The role requires an active RN license in Colorado, case management and supervisory experience, strong communication, vendor management, and willingness to participate in on-call rotation.
