LPN, Discharge Lounge – Patient Flow Champion

Boston Medical Center in Boston seeks an experienced Licensed Practical Nurse (LPN) for the Discharge Lounge to support safe, efficient patient transitions from hospital to home or another level of care. You will coordinate patient flow, monitor patients awaiting transportation, assist with medications, and collaborate with inpatient units, pharmacy, transportation, and the interdisciplinary team to ensure a smooth discharge while maintaining safety and satisfaction.
