Post-Discharge Care APP — Telehealth Innovator

Devoted Health is seeking an experienced Advanced Practice Provider (APR N/PA) for the Veterans TOC program. The role combines initial post-discharge visits with virtual urgent-care and chronic-care management, within a highly interdisciplinary team. The ideal candidate has 5+ years in geriatric care, strong acute and chronic disease management skills, and licensure or eligibility to practice in required states within 90 days of hire.
