Home Health Licensed Practical Nurse

Provide direct in-home nursing care to pediatric and adult patients per physician orders Administer medications, treatments, and wound care; monitor vital signs and patient status Develop, implement, and update individualized care plans Educate patients and families on disease management, medications, and home care techniques Document all care accurately in electronic medical records and maintain compliance with policies Collaborate with physicians, therapists, and the care team to coordinate services Respond to changes in patient condition and escalate concerns promptly Ensure infection control and safety standards in the home environment Support patient comfort, dignity, and quality of life through compassionate, family-centered care LPN clinical assessment Medication administration Wound care Pediatric nursing basics IV therapy and injections Care planning and documentation Home health regulations and compliance Electronic medical records (EMR) use Patient and family education Infection control and safety Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN license in state of practice Current BLS/CPR certification Home health or pediatric experience preferred Valid driver’s license and reliable transportation
