Role Description:
The Supports Coordinator role is a full-time, on-site position located in Philadelphia, PA. This role provides day-to-day support to individuals receiving behavioral health services, including assisting with service coordination, documenting interactions, and ensuring support plans are followed. The Supports Coordinator will communicate regularly with clients, families, and clinical staff to address needs, clarify information, and help resolve issues that may arise. Responsibilities include monitoring client progress, assisting with problem-solving, and supporting access to resources that promote recovery, independence, and quality of life.
Qualifications:
A bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science. Two years of college (including earning an associate degree) with 12 eligible credits AND two years’ experience in a position performing paraprofessional case management functions 12 eligible credits AND three years’ experience in a position performing paraprofessional case management functions**- experience can be calculated through the completion of acceptable social science/social service courses at the rate of 30 credits equaling one year of experience.
