This role involves coordinating clinical assessments, implementing nursing interventions, and ensuring compliance with healthcare standards.
Responsibilities:
Evaluate patients to develop and adjust individualized care plans based on assessments and physician diagnoses.
Perform therapeutic nursing interventions, including managing dialysis treatments utilizing Fresenius T Machines.
Document patient care accurately, adhering to standards of practice, acuity levels, and clinical priorities.
Collaborate with physicians and healthcare team members to ensure optimal patient outcomes.
Advocate for evidence-based practices and support the adoption of innovative care approaches.
Manage patient care in a fast-paced, acute dialysis setting, ensuring safety and quality standards.
Qualifications:
- Require a minimum of 1 year of acute dialysis experience, with a strong preference for telemetry expertise.
- Demonstrate proficiency with Fresenius 2008T Hemodialysis Machines and EPIC EHR systems.
- Possess an ASN degree; BSN preferred.
- Hold active BLS certification (AHA) and a valid NJ State Professional Registered Nurse license or multi-state compact license.
- Knowledge of renal physiology and hemodialysis procedures.
