Job Details: Schedule: Monday-Friday, approximately 8:30 AM - 4:30 PM

Location: Wallingford, CT 06492 Float / Coverage Area: Middletown, CT and surrounding towns

Contract Length: 14 Weeks

Pay Range: $50.00 to $57.00/hour*Mileage Reimbursement Rate (if applicable): $0.52/ mile*Pay offered is based on experience, expertise, credentialing, and education.

Duties: Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and physician orders. Provide skilled home care physical therapy services focused on improving strength, balance, mobility, endurance, safety, and functional independence. Conduct ongoing patient assessments and modify treatment plans as appropriate to achieve optimal outcomes. Educate patients, families, and caregivers on therapeutic exercises, fall prevention, mobility techniques, safety strategies, and home exercise programs. Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines. Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate patient care. Maintain productivity standards while delivering exceptional patient-centered care. Participate in care conferences and contribute to individualized patient care planning.

Requirements: Active Physical Therapist (PT) License in the State of Connecticut. Graduate of an accredited Physical Therapy program. Strong clinical assessment and treatment planning skills. Excellent communication, organization, and time-management abilities. Valid driver's license, reliable transportation, and current auto insurance.
