Key responsibilities:
Coordinate care across multiple providers and settings for patients with complex medical and social needs
Provide chronic disease education and perform medication reconciliation for patients
Implement programs aimed at improving health outcomes and reducing unnecessary healthcare utilization
Required qualifications:
Bachelor's Degree in Nursing (BSN) is required
Minimum of 2 years of experience in clinical nursing is required
Current Maryland RN License or multi-state/compact license is required
Experience in managed care is preferred
Experience in a community-based acute care setting is preferred
