Primary Care Home Health Registered Nurse

Provide skilled nursing care to patients in their homes and community settings. Assess health conditions, monitor treatment progress, educate patients and caregivers, coordinate with the care team, and document care while helping patients access community resources. Provide skilled nursing care in patients’ homes and community settings. Assess patients’ conditions, monitor disease progression, and evaluate treatment responses. Carry out established plans of care. Educate patients and caregivers to support recovery, independence, and disease prevention. Coordinate care with physicians and interdisciplinary care team members. Document care and connect patients and families with community resources; direct LPNs and supervise aide care as appropriate.
