Hospice Home Health Registered Nurse

CarePartners is seeking a compassionate Registered Nurse Hospice Homecare professional to deliver high-quality, patient-centered end-of-life care in the comfort of patients’ homes. In this field-based role, you will conduct comprehensive assessments, develop and implement individualized care plans, manage pain and symptoms, and provide hands-on nursing care and education to patients and families. Collaborating with an interdisciplinary team, you’ll ensure coordinated, culturally sensitive care that reflects our values of compassion and community health improvement. Deliver skilled nursing care to hospice patients in their homes, focusing on comfort and quality of life. Perform comprehensive assessments, develop individualized care plans, and adjust treatment based on patient needs. Administer medications, manage symptoms, and provide wound and IV care as appropriate. Educate patients and families on disease progression, symptom management, and available support resources. Collaborate closely with physicians, social workers, chaplains, and aides as part of an interdisciplinary team. Document all care accurately and timely in the electronic health record system. Participate in on-call rotations and respond to urgent patient needs in the home setting. Support families through end-of-life decisions and provide bereavement resources. Advocate for patient wishes while upholding hospice philosophy and regulatory standards. Contribute to quality improvement and community health initiatives aligned with CarePartners’ mission.
