Job Description & Requirements Specialty: Occupational Therapist Discipline: Therapy Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Occupational Therapist (OT) - Home Health Location: Fernley, NV 89408 Patient Care Setting: Home Health Schedule: Full-Time | 8 Hours Per Day Travel: Local Travel Required Position Overview We are seeking a dedicated and compassionate Occupational Therapist (OT) to provide skilled therapy services to patients in the home health setting. The Occupational Therapist will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions designed to improve functional independence, safety, and quality of life. This role involves extensive collaboration with physicians, caregivers, and interdisciplinary healthcare teams to ensure exceptional patient outcomes.
Key Responsibilities: Perform comprehensive occupational therapy evaluations and clinical assessments to identify patient needs and determine appropriateness for therapy services. Develop, implement, revise, and manage individualized Plans of Care (POC) based on physician orders and patient-specific goals. Assess and treat deficits related to activities of daily living (ADLs), functional mobility, strength, range of motion (ROM), coordination, cognition, sensory processing, and psychosocial functioning. Recommend and train patients in the safe use of adaptive equipment, assistive devices, splints, and environmental modifications. Evaluate self-care, work, leisure, and daily living activities to improve patient independence and safety. Fabricate and maintain adaptive equipment and hand splints as needed. Assess physical, cognitive, psychosocial, sensory-motor, and environmental factors affecting patient performance. Educate patients, caregivers, and families on therapeutic techniques, adaptive strategies, home safety, and equipment usage. Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary healthcare team. Maintain accurate and timely documentation, including evaluations, treatment notes, physician communications, and care plan updates. Ensure compliance with infection control standards, Medicare guidelines, and agency policies. Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings.
Required Qualifications: Active Occupational Therapist (OT) License in the State of Nevada. Graduate of an accredited Occupational Therapy program with a Bachelor's Degree in Occupational Therapy. Minimum 2 years of Occupational Therapy experience in an acute care, rehabilitation, or related clinical setting. Current CPR Certification. Valid driver's license. Reliable transportation and proof of automobile insurance. Strong clinical assessment, treatment planning, and documentation skills. Ability to travel locally within the assigned service area.
