In-Home NP/PA Clinician Flexible Travel & Diagnostics

Signify Health in the United States is seeking an in-home clinician to perform visits for health plan members, including medical history reviews and physical evaluations in the member's home. You will use an iPad with a streamlined clinical workflow, travel locally to licensed states as needed, and provide diagnostic and preventive services such as spirometry and diabetic eye exams. This is a part-time role with 12 hours per week.
