Home Health Speech-Language Pathologist Case Manager

Provide home health speech-language pathology care, assessing patient needs and implementing evidence-based interventions to support communication, swallowing, self-care, and quality of life. Coordinate care with patients, families, and interdisciplinary teams; provide education; document services; and contribute to quality and patient safety initiatives.
