Home-Based Primary Care - Physician Assistant or Nurse Practitioner

Position Highlights: Home-based primary care model focused on high-risk and complex patients Manage both acute and chronic conditions in the patient's home Collaborate with interdisciplinary care teams to coordinate care Build long-term relationships with patients and caregivers Opportunity to provide comprehensive, value-based care outside of a traditional clinic setting Independent practice environment with strong clinical support Required QualificationsCurrent NP or PA licensure and board certification Active, unrestricted state license (or ability to obtain) Active DEA license or ability to obtain Active driver's license and reliable transportation Ability to conduct home visits and travel throughout the assigned territory Experience caring for adult, geriatric, or medically complex patients Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
