Working in patients' homes allows you to continually adapt treatment plans that best suit individual needs and environments. Our supportive work culture encourages collaboration among healthcare professionals, ensuring that all aspects of patient care are addressed holistically.Key ResponsibilitiesConduct comprehensive evaluations to assess patients' physical abilities and create tailored rehabilitation plans.Communicate evaluation findings effectively to patients and their families, involving them in the goal-setting process.Implement treatment protocols proficiently based on assessment findings and adapt them as patient conditions evolve.Consult with other healthcare professionals when necessary, ensuring a multidisciplinary approach to patient health.Monitor and document changes in patient conditions, adjusting care plans in response to these developments.Ensure accurate and timely documentation of patient care activities, including evaluations, progress notes, and discharge summaries that reflect the patient's course of treatment.Maintain an effective schedule and manage your time efficiently to meet productivity expectations.Engage in community outreach and marketing initiatives to promote our home care services.Contribute to departmental goals and professional development through participatory activities and continuing education.Patient Population / Client FocusThe patients you will care for are diverse, ranging from those recovering from surgical procedures to individuals managing chronic pain and mobility impairments. Your role will be vital in empowering them to regain independence and enhance their functional abilities in their everyday lives.QualificationsBachelor's degree in Physical Therapy from an accredited institution.Current state license to practice Physical Therapy in Florida.Certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.A genuine commitment to patient care, with the ability to work independently while collaborating with a multidisciplinary team.Preferred ExperienceA minimum of 2 years of experience as a Physical Therapist although fresh graduates with strong clinical internships and educational backgrounds are encouraged to apply. Familiarity with home health documentation and electronic health records is a plus.Compensation and BenefitsThe salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience.
