Salary: $240,000 $270,000 + Performance-Based Incentives
Key Responsibilities:
- Conduct H&Ps, periodic reassessments, and develop medical plans
- Discuss care plans, diagnoses, and prognosis with participants and caregivers
- Participate in intake evaluations and family/team conferences
- Collaborate with interdisciplinary teams on chronic disease management
- Provide leadership and strategic input as part of the senior care model
- Order labs, diagnostics, and perform physical exams
- Participate in on-call rotation (as required)
- Educate participants, families, and team members on treatment options
- Care delivered across multiple settings where participants reside
Ideal Candidate Profile:
- Active FL Physician License RequiredBoard Certified in Internal or Family Medicine RequiredBoard Cert. in Geriatrics PreferredCDS & DEA Registration Required
- Supervisory/Leadership Experience Required 1+ Year of Experience with Elderly or Frail Populations Required
- Strong skills in physical assessment & chronic condition management
- Passion for innovative, patient-centered senior care
