Hospital Admissions Liaison We are hiring a fully remote Hospital Admissions Liaison to support weekend admissions for our portfolio of long-term care and skilled nursing facilities. You will work alongside our other Hospital Liaisons reviewing and coordinating referrals from hospitals throughout the St. Louis region, Springfield area, and surrounding communities.
Qualifications: Long-term care or skilled nursing admissions experience required Must be able to confidently read and interpret hospital referrals, medical records, and clinical documentation Nursing or clinical background is recommended, but not required Strong computer skills and ability to navigate multiple referral systems Organized, detail-oriented, and able to work independently Must reside in IL or MO
Responsibilities:
  • Review and screen incoming hospital referrals Identify key clinical and admissions information Create a clear, concise synopsis of each referral Route referrals to the appropriate facilities and admissions teams Communicate with facility teams to help coordinate weekend admissions
  • Schedule & Pay
  • Friday, Saturday & Sunday Three 12-hour shifts / 36 hours per week Full-Time 100% Remote $23$25 per hour
This is a great opportunity for someone with long-term care admissions experience who resides in Illinois or Missouri and is confident reviewing referrals and is looking for a consistent, fully remote weekend schedule.
