Status: Salaried W-2 position (Full‑Time or Part‑Time) -$1,500-$2,000weekly Productivity expectation of 30 patient visits per week, with an additional per-visit rate of approximately$65–$70for visits completed above that threshold

THE GROWTH YOU HELP PROVIDE AS A HOME HEALTH REGISTERED NURSE

What You'll Do: Perform in‑home skilled nursing visits - SOCs, recerts, routine skilled visits, discharges; complete thorough assessments, implement plan of care, and educate patients/families. Document accurately and on time in the EMR (assessments, care provided, orders, signatures) to support billing, compliance, and continuity. Coordinate care & communicate with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current. Monitor progress & escalate concerns (changes in condition, safety risks, non‑compliance) to prevent adverse events and avoid unnecessary hospitalizations. Support compliance & quality - follow policies, payer requirements, and home health standards; participate in QA and care plan updates.
