Advanced Practice Pharmacist: Medication Management & Patient Care (Unlimited PTO)

Akido Labs, Inc. seeks an Advanced Practice Pharmacist to provide comprehensive medication management across primary and specialty practices. You will collaborate with physicians, APPs, nurses, and care teams to optimize therapy and support evidence-based interventions. You will conduct patient assessments, optimize regimens, document in the EHR, and participate in quality improvement and population health initiatives focused on chronic disease management and patient safety.
