Vernon area. In this field-based role, you will provide practical nursing care in patients’ homes, monitor changes in condition, administer medications and treatments, assist with symptom and pain management, and document care accurately. You will coordinate with hospice nurses and the interdisciplinary team, communicate updates, and help patients and caregivers understand care plans.
