Community Wellness CoachThe Community Wellness Coach operates within Broward Health Point to provide support, guidance, and advocacy to patients and their families throughout their healthcare journey. Acting as a vital liaison between patients and healthcare providers, the CWC plays a crucial role in ensuring access to quality care, personalized patient education, and coordinated services with a focus on individual health and wellness.
