Hospice Registered Nurse Case Manager

Hartford Healthcare seeks a Float Hospice RN to provide compassionate weekend care across our hospice network. In this role, you’ll perform comprehensive assessments, manage complex symptoms, and coordinate interdisciplinary care as a case manager for patients and families. You’ll educate caregivers on end-of-life needs, document in the EHR, and respond to urgent clinical changes. Provide weekend hospice nursing care to patients in home or facility settings as a floating RN. Complete comprehensive patient assessments and develop individualized care plans. Administer medications, treatments, and symptom management per hospice protocols. Serve as case manager, coordinating care with physicians, social workers, aides, and chaplains. Educate patients and families on disease process, medications, and end-of-life care. Document all care accurately in the electronic health record in a timely manner. Respond to urgent changes in patient condition and adjust care plans as needed. Participate in interdisciplinary team meetings and contribute to quality improvement.
