Hospice Home Health Registered Nurse

Hartford Healthcare seeks a compassionate Weekend Hospice RN to provide home-based end-of-life care to patients across our Connecticut service area. In this field-based role, you’ll complete holistic assessments, manage complex symptoms, administer medications, and deliver skilled nursing interventions. You’ll collaborate closely with an interdisciplinary hospice team, educate and support families, and ensure individualized care plans honor patient goals. Provide home-based hospice skilled nursing care on weekends Perform comprehensive patient assessments and ongoing reassessments Develop, implement, and update individualized hospice care plans Administer medications and treatments, including pain and symptom management Educate and support patients and families on disease process and end-of-life care Collaborate with physicians, social workers, chaplains, and other team members Document care accurately in the electronic health record Participate in on-call rotation and respond to urgent patient needs Ensure compliance with hospice regulations, policies, and standards Communicate changes in patient condition promptly to the interdisciplinary team Patient assessment IV and medication administration Care planning Electronic health records (EHR) documentation Pain and symptom management Wound care Care coordination Patient and family education Hospice regulations and compliance Driving and time management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in CT BSN preferred BLS certification Hospice and Palliative Care Nursing Certification (CHPN) preferred Valid driver’s license and reliable vehicle Home health or hospice experience preferred
