Home Health Care Coordinator (RN or MSW)

Volunteer State Health Plan, Inc. is seeking a CHOICES Care Coordinator to travel to members' homes, assess needs, and coordinate services to promote good health and independence. You will work with a team of clinicians and social supports to deliver person-centered care. The role requires an RN licensed in Tennessee or a MSW with an active unrestricted license, excellent customer service, and strong time management.
