Geriatric Physical Therapist Assistant

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality therapy services for seniors in skilled nursing and post-acute care settings. Under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, assist with gait and balance training, and document patient progress. Collaborate within an interdisciplinary team in a patient-centered, respectful environment focused on integrity and quality outcomes. Provide physical therapy interventions under the supervision of a licensed Physical Therapist Implement and progress therapeutic exercise, gait, balance, and functional mobility programs Assist with patient evaluations by gathering data and observing functional status Accurately document treatments, patient responses, and progress in the medical record Educate patients and families on home exercise programs and safety strategies Collaborate with an interdisciplinary care team to support patient-centered goals Ensure treatments comply with physician orders, PT plan of care, and regulatory standards Maintain safe treatment environments and proper use of equipment Participate in case conferences, in-services, and ongoing clinical education Support productivity, documentation timeliness, and quality measures for the facility Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute documentation Use of assistive devices and durable medical equipment Patient and family education Functional outcome measurement Infection control and safety procedures Electronic medical record (EMR) proficiency Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state licensure or certification as a Physical Therapist Assistant (PTA) Graduation from an accredited Physical Therapist Assistant program Current Basic Life Support (BLS) or CPR certification Experience or clinical rotations in geriatric or skilled nursing settings preferred
