Job Description: Job Description Apply through this link to be considered: -health-clinician Mental Health Clinician NYCWe're seeking a licensed mental health professional to join a multidisciplinary community-based team supporting individuals experiencing homelessness or unstable housing. This is a field-based role focused on outreach, engagement, housing transitions, and ongoing support in the community.
What you'll do:
  • Conduct outreach and build trusting relationships with individuals in the community
  • Provide short-term counseling, crisis intervention, and supportive services
  • Complete psychosocial and risk assessments and develop individualized care plans
  • Support participants before, during, and after transitioning into housing
  • Connect individuals with healthcare, housing, and community resources
  • Coordinate with hospitals, outreach organizations, social services, and other community partners
  • Help participants navigate appointments and overcome barriers to care
  • Support case managers and peer staff as part of a multidisciplinary team
  • Document progress and contribute to team meetings and care planning
What we're looking for:
  • Master's degree or higher in social work, mental health counseling, psychology, nursing, or a related field
  • Appropriate professional licensure required
  • Experience with individuals experiencing homelessness, mental health conditions, or substance use disorders is a plus
  • Strong counseling, crisis management, de-escalation, and community outreach skills
  • Person-centered, trauma-informed, and harm-reduction approach
  • Strong communication, documentation, and collaboration skills
  • Comfortable working in the field with flexible hours and occasional on-call coverage
