Community Health Advocate for Moms & Babies

CHAS Health is seeking a dedicated team member to improve maternal and infant health access within the communities we serve. You will conduct intakes, assess needs, and connect patients with appropriate resources under the First Steps guidelines. Responsibilities include coaching families, coordinating with multidisciplinary teams, performing home visits when needed, and supporting outreach activities to promote healthy birth outcomes. A valid driver's license is required.
