Home Health Registered Nurse

Hartford Healthcare seeks a compassionate Weekend On Call Visiting Nurse to provide high-quality home-based care to patients across Connecticut. In this part-time role, you will conduct skilled nursing visits, triage calls, and respond to urgent patient needs on weekends. Responsibilities include comprehensive assessments, wound and IV care, medication management, and coordination with physicians and the interdisciplinary team. You’ll document in our EMR, educate patients and families, and contribute to care plans in a supportive, team-focused culture dedicated to excellence and innovation. Provide weekend on-call home health nursing visits to assigned patients Perform comprehensive patient assessments and monitor ongoing status Administer medications, wound care, and IV therapies per orders Triage patient calls and respond to urgent needs and changes in condition Collaborate with physicians and interdisciplinary team to update care plans Educate patients and caregivers on disease management and home care Accurately document all care and communication in the EMR system Ensure compliance with home health regulations, policies, and safety standards Participate in case conferences and handoffs to weekday care teams Travel between patient homes across the service area using personal vehicle Patient assessment IV therapy and wound care Care planning and documentation (EMR) Medication administration and reconciliation Home health regulations and compliance Infection control Care coordination and communication Crisis and triage management Time management and scheduling Patient and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in CT BLS certification Home health or hospice experience preferred Valid driver’s license and reliable transportation
