Hospice Home Health Registered Nurse

Hartford Healthcare is seeking a Hospice On Call Visiting Nurse to provide compassionate, expert care to patients in their homes on a 7 days on / 7 days off schedule. In this role, you’ll respond to after-hours hospice needs, perform comprehensive assessments, manage symptoms, administer medications, and update care plans in collaboration with the interdisciplinary team. You will educate patients and families, ensure continuity of care, document accurately in the EMR, and coordinate with physicians and caregivers. Provide after-hours hospice visits in patient homes on a 7 on / 7 off schedule Perform comprehensive nursing assessments and ongoing evaluations Manage pain and symptoms, administer medications, and provide skilled nursing care Update and implement individualized hospice care plans Collaborate with physicians and the interdisciplinary hospice team Educate patients and families on disease process, medications, and end-of-life care Ensure accurate and timely documentation in the electronic medical record Coordinate care and communicate changes in condition to the care team Support patients and families with compassionate communication and advocacy Adhere to hospice, safety, and regulatory standards and policies Hospice nursing Home health nursing Patient assessment Pain and symptom management IV and medication administration Electronic medical records (EMR) documentation Care planning and coordination Patient and family education Crisis intervention Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in CT Current BLS certification Hospice and Palliative Care Nursing Certification (preferred) Valid driver’s license and reliable transportation Experience with EMR systems
