Remote Bilingual Care Navigator

Alignment Health is seeking a remote Care Coordinator to support its Special Needs Program (SNP) case management team. This role helps members with complex health needs by collaborating with the RN Case Manager and interdisciplinary care team. You will conduct outreach, coordinate referrals and prior authorizations, monitor hospital/SNF discharges, and maintain accurate member records while ensuring confidentiality in a fast-paced remote environment.
