Home Health and Hospice RN Clinical Manager

As an RN Clinical Manager at Gentiva, you will lead a team delivering compassionate, clinically advanced home-based care. You’ll oversee nursing operations, coordinate interdisciplinary care, and ensure quality, safety, and regulatory compliance for home health and hospice patients. This role combines leadership and hands-on clinical oversight, guiding staff in developing care plans, managing complex cases, and supporting families. Oversee day-to-day clinical operations for home health and hospice services Supervise, mentor, and schedule RN and interdisciplinary care teams Ensure adherence to clinical protocols, regulatory standards, and Gentiva policies Coordinate and review patient assessments, care plans, and outcomes Collaborate with physicians, families, and community partners to optimize care Monitor quality metrics, patient satisfaction, and clinical documentation accuracy Lead staff training, performance evaluations, and competency development Support onboarding of new clinicians and promote a culture of compassion and dignity Participate in case conferences and complex care planning Contribute to process improvements and best practices in home-based care Clinical care management Home health nursing Hospice and palliative care knowledge Regulatory compliance (CMS, Joint Commission) Care planning and case management Electronic health records (EHR) documentation Staff supervision and performance management Quality improvement and outcome monitoring Patient and family education Interdisciplinary team coordination Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active, unrestricted Registered Nurse (RN) license BLS certification ACLS preferred Prior home health or hospice experience preferred Supervisory or charge nurse experience preferred
